Women in Transition OKC Volunteer Agreement

1. Make a one-year commitment to the small-group process.
{{item}}
2. Meet with the Neighbor, as a group, a minimum of twice per month for 1 1/2 hours each meeting.
{{item}}
3. Contact the Neighbor weekly (by phone, text or email) to see how he/she is doing.
{{item}}
4. Offer no money to the Neighbor (that is not earned through an approved activity by WIT staff).
{{item}}
5. Show up and be on time for my scheduled meetings, knowing that consistency is one of the most important ways to build trust and relationship.
{{item}}
6. Notify the Neighbor and my other group members in advance if I am unable to keep a scheduled mentoring session.
{{item}}
7. Give my Neighbor my undivided attention during our time together, and make and accept no phone calls or texts while I am meeting with my Neighbor (unless it is an emergency).

8. Keep discussions with my Neighbor confidential, unless her safety or well-being is at risk.
{{item}}
9. Seek to establish a unified small-group family who speaks as One Voice when evaluating choices, decisions and options in group discussions.

10. Establish consistent dialogue with the other group members to communicate observations and concerns about the Neighbor or recommendations for future action/activities.

11. Pray for the Neighbor and all group members regularly.
{{item}}
12. Continue to practice spiritual disciplines to encourage my own spiritual and emotional development and better serve as an example of Christ to my Neighbor.

13. Communicate with the Program staff when I need assistance, clarification, or am having difficulty with any aspect of the small-group relationship.

14. Attend at least one of the Mentor Roundtables during the next 12 months.
{{item}}
15. Notify the Program Manager as soon as possible: -- of any significant change(s) in the Neighbor; and -- any time the Neighbor does not keep her scheduled meeting.

16. Notify the Program Manager of any changes in my employment, address, email or telephone number.

17. PREPARE FOR THE NEXT TIME EVERY TIME. Preparing for the next time after each WIT family meeting helps everyone stay on the same page and know when and where the next WIT family meeting will be.

Stand in the Gap Media Release Form

I hereby grant Stand in the Gap permission to use my likeness in a photograph in any and all of its publications, including but not limited to Stand in the Gap's printed, social media related, and other digital publications. I understand and agree that any photograph using my likeness will become property of Stand in the Gap when submitted to staff or when the photograph is taken by a staff member or representative of Stand in the Gap.

I acknowledge that my participation is voluntary, and I will receive no financial compenstion. I authorize Stand in the Gap to edit, alter, copy, publish, and distribute photos for purposes of publicizing Stand in the Gap's program or other related purposes. I waive the right to royalties or any other compensation arising or related to the use of the photograph. I release Stand in the Gap from all claims and demands which may arise from those acting on behalf of myself or on behalf of my estate.
